Finding your “Medical Home.”
A medical home is a place of service where a patient can go to have his or her medical questions answered and health needs met without the high costs of an ER visit. The free clinic medical homes of the RFCC seek to keep their patients healthy through quality primary care services and a continued relationship.
Medical homes are especially important for patients who suffer from chronic medical problems, who are at high risk for health problems, or use the emergency room more than once a year.

Do you qualify for free clinic care?
If you meet both of the following, you may be able to receive care from one of our free clinics: 1. You do not have health care insurance or assistance. (This includes not having Medicare, Medicaid, VA benefits, or private insurance.) 2. Your total household income is below our limit of the 200% federal poverty level.
Once determined eligible, most patients qualify for services for one year.

What to bring to your first visit?
Photo Identification
Proof of income documentation for entire household. This can be:
The most recent 2 months of pay stubs, social security checks, or unemployment checks, most recent tax return OR
A letter from an employer with hire date, wages, and average hours worked OR
A notarized letter showing how expenses are met if you have no income
Any medications you are currently taking (in actual pill bottles).
Please print and fill out the eligibility form and bring it with you to your first appointment.
